Hepatocellular Carcinoma

Clinical History:
65 year old woman being evaluated for liver transplant. The patient has a history of hepatocellular carcinoma status post RFA in 2005.

Diagnosis:
Hepatocellular carcinoma.

MR Technique:
3T (Siemens Verio) using body matrix coil: Image 1: Axial pre contrast VIBE (TR/TE 3.9/1.3, slice thickness 4 mm, scan time 11 sec); Image 2: VIBE post contrast phase 1; Image 3:VIBE post contrast phase 2; Image 4 VIBE post contrast phase 3. The contrast used was 0.1 mmol/kg of gadolinium chelate.

Imaging Findings:
A round lesion measuring approximately 2 x 2.2 cm is identified in segment III. The lesion appears hypointense to liver on T1, slightly hyperintense on T2 weighted images and shows restricted diffusion (Diffusion weighted images are not included). On the post contrast images the lesion shows homogeneous arterial enhancement with washed out of contrast on the delayed images. Mild capsular enhancement is seen on delayed images.
