Left Ovarian Dermoid

Clinical History:
26 yr old woman presented with pelvic pain.

Diagnosis:
Left ovarian dermoid

MR Technique:
3T (Siemens Verio) using body flex coil; Image 1: In phase (TR/TE 5.5/2.5 ms, slice thickness 3 mm, scan time 22.57 sec; Image 2: Out of phase (TR/TE 5.5/3.7 ms, slice thickness 3 mm, scan time 22.57 sec; Image 3: T1 sagittal VIBE fat saturated after injection of 0.1 mmol/kg of gadolinium chelate (TR/TE 4.6/2 ms, slice thickness 3 mm, scan time 28.7 sec); Image 4: T2 coronal fat saturated (TR/TE 1200/111 ms, slice thickness 1 mm, scan time 5 min 29 sec).

Imaging Findings:
A 7.9 x 4.8 x 6.5 cm lesion is seen in the expected location of the left ovary. This lesion contains a cystic appearing component which is hyperintense on T2 and hypointense on T1.  Within this cystic component several nodules are present which are largely hyperintense on nonfat saturated images and hypointense on the fat saturated images.  The largest of these nodules measures 2.7 x 1.9 cm. Adjacent to the cystic component is another fat containing nodule which also likely has a calcification. No enhancement is appreciated within this lesion.
