Clinical History:
A 56 year-old man with back pain

Diagnosis:
T12, L1 vertebral metastases (renal cell carcinoma)

MR Technique:
Sagittal TSE 4 mm pre-contrast T1-, STIR and post-contrast T1-weighted images are presented, acquired at 1.5 T on a Siemens Avanto MR system. Sequence parameters were TR/TI/TE = 665/7.9, 2520/160/71 and 490/7.9, with scan times of 3:14, 3:16 and 5:16. The post-contrast scan was acquired with fat saturation.

Imaging Findings:
Sagittal TSE 4 mm pre-contrast T1-, STIR and post-contrast T1-weighted images are presented, acquired at 1.5 T on a Siemens Avanto MR system. Sequence parameters were TR/TI/TE = 665/7.9, 2520/160/71 and 490/7.9, with scan times of 3:14, 3:16 and 5:16. The post-contrast scan was acquired with fat saturation.

There is confluent extension of enhancing soft tissue into the pre- and paravertebral soft tissues at L1 with additional involvement of the T12 vertebral body, all findings consistent with further spread of metastatic disease. Homogeneous fatty marrow replacement is seen in the upper lumbar spine in areas uninvolved by metastatic disease, consistent with prior radiation therapy.
