Thoracic dural AV fistula

Clinical History:
58 yr old woman presented with back pain. An MRI of the thoracic and lumbar spine was obtained.

Diagnosis:
Thoracic dural AV fistula

MR Technique:
3 T scanner (Siemens Verio) with spine coil. Image 1: Sagittal T2 turbo spin echo (TR/TE 3000/106 ms, slice thickness 3 mm, scan time 3 min 8 sec); Image 2: Sagittal T2 STIR (TI 220 ms, TR/TE 4000/54 ms, slice thickness 3 mm, scan time 4 min 42 sec); Image 3: Sagittal T1 (TR/TE 600/94 ms, slice thickness 3 mm, scan time 4 min 10 sec); Image 4: Axial T2 (TR/TE 4260/82 ms, slice thickness 4 mm, scan time 5 min 34 sec).

Imaging Findings:
Images demonstrate multiple serpentine flow voids posterior to the spinal cord extending from T8-9 to T12-L1 consistent with a dural AV fistula.  Axial T2 images demonstrate prominent abnormal vessels in the paraspinal space bilaterally at T9-10. No cord edema was identified.
